The series, released on February 25, examines the shocking reality of Perry’s accidental ketamine overdose 

and the individuals who were ultimately charged in connection with his passing.

Matthew Perry’s Cause of Death

The official autopsy report confirmed that Perry died from the acute effects of ketamine. Drowning, coronary artery disease, and buprenorphine—a medication used to treat opioid addiction—were listed as contributing factors. His death was ruled accidental.

The coroner’s report noted that the level of ketamine in Perry’s system was dangerously high and would have caused severe cardiovascular overstimulation and respiratory depression. As a result, he likely lost consciousness and drowned in his hot tub.

Perry had been receiving ketamine infusion therapy, a treatment often used for depression and anxiety. However, the coroner clarified that the ketamine found in his system could not have been from his last medically supervised session, which had occurred more than a week prior. The drug’s half-life is only a few hours, meaning the ketamine in his body had to have come from another source.

Adding to the mystery, no drugs or drug paraphernalia were found near the pool where he was discovered, and authorities found no evidence of foul play.

The Investigation and Arrests

Following Perry’s death, a criminal investigation uncovered an underground drug network that had been supplying ketamine to the actor and others. Authorities later charged five individuals, including Perry’s personal assistant and two doctors, with offenses related to his overdose. The investigation revealed that Perry’s assistant, Kenneth Iwamasa, had administered ketamine injections to the actor on the day of his death, despite lacking medical training.

According to reports, Perry had asked Iwamasa to inject him with ketamine three times that day. The first injection occurred at approximately 8:30 a.m., followed by a second dose just after noon while Perry was watching a movie. Around 40 minutes later, Iwamasa administered a third dose before leaving to run errands. When he returned later in the afternoon, he found Perry unresponsive in the hot tub.

The investigation also found that two doctors had been illegally prescribing controlled substances to Perry, further fueling concerns about how he obtained such high doses of ketamine.

Matthew Perry’s Final Hours

On October 28, 2023, Perry was discovered in his Pacific Palisades home’s jacuzzi by his assistant. Emergency responders arrived at the scene at 4:07 p.m. after receiving a 911 call regarding a “medical emergency,” but Perry was already deceased by the time they arrived.

His assistant had reportedly repositioned him so that his head was above water before firefighters arrived. The Friends star had moved into the home just weeks before his death, having completed renovations. Close friends said Perry had referred to the home as “heaven” and had seemed to be in a good place emotionally.

Matthew Perry’s Struggles and Sobriety Journey

Perry had battled substance abuse for decades, openly discussing his struggles in his 2022 memoir, Friends, Lovers, and the Big Terrible Thing. He revealed that he began drinking at age 14 and later became addicted to painkillers after a jet-ski accident in 1997. Over the years, he sought treatment more than 15 times, but his addiction had already taken a toll on his health.

In 2018, Perry suffered a life-threatening health crisis when his colon burst due to opioid overuse. He spent five months in the hospital, two weeks in a coma, and underwent multiple surgeries. Doctors gave him only a 2% chance of survival.

Despite these challenges, Perry appeared to have turned a corner in recent years. In a 2022 interview, he told PEOPLE that he felt “safely sober” and was optimistic about his future. However, he also made a haunting admission in his memoir: “If I did die, it would shock people, but it wouldn’t surprise anybody.”

Matthew Perry’s Legacy

Matthew Perry was just 54 years old when he passed away. He left behind a legacy not only as the iconic Chandler Bing from Friends, but also as an advocate for addiction recovery. He dedicated much of his later life to helping others struggling with substance abuse, even opening a sober living facility for men.

His death serves as a tragic reminder of the dangers of addiction, even for those who have worked hard to overcome it.
